New Layout issue

 I like this google form as it shows the netire form... can you iframe yourlike t switch scren for address, etc... do you have setting where it shows entire form like this google form on link above?

I checked your account and it seems that you have changed your form layout from new layout (as you mentioned above each screen for each field) to legacy layout.

Since you are a new user, from now on forms you created will be new layout unless you specifically changed it to earlier version.
