When you first fill out our SAU #21 application form, the calendar button on the date field doesn't work. You click it and nothing happens. But when you as a user go back and edit your submission, it does work. How do we fix that?

I tested your form and I am unable to reproduce the error.
So lets try some troubleshooting:
1. Are you embedding this form? Or do you use it stand alone?
2. Can you try emptying your cache, form fills, cookies, etc?
3. Can you try using another web browser and see if the problem persists?
4. Please specify exactly which date field?
