I am creating an application. I need to know the Race of applicants. How can i structure this?

 American Indian/Alaska Native
     Asian: Please Specify: Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Phillipine Islands, Thailand, and Vietnam.
     Black or African American
     Native Hawaiian/Other Pacific Islander: Please Spcify: Hawaii, Guam, Samoa, or other Pacific Islands.
     White
     Some other race 
     More than one race: Please specify: 
     Declined
     Unavailable/Unknown

Or, if you would like to do it yourself, here are the steps:

1. Search for dynamic dropdowns on the left and drag it onto your form.

2. Configure the widget. Click on the wizard (magic wand icon). And under Values enter your race data. Each sub menu is created if you indent the list. For example by two blank spaces. 

Note that the widget has instructions of how to configure it.
