I have checked the account related to the form you have provided and I can see it was registered under the same email address you used to open this thread, you could just request a password change and use your email address, you should receive the email with the instructions.
To reset your password, you only need to go to the JotForm page and click on the login link, once there click on the forgot password link:
Enter the email address to receive the instructions:
