Add Reload Link/Button

dear sir, i am trying to create a simple profile questionnaire.

if score of <4, they will be channeled to page 2 (profile 1) and if score > 4, chanelled to page 3 (profile 2).

at both pages 2 & 3, how do i create a re-do button - that reloads, to enable user to clear / re-attempt? thanks much.

You can try to add a link to your form that will reload/refresh the form. To add a link to your form, you can use the Text field.

Click on the Link icon to add a link to the field.

Add the link to your form. 

Then ensure to save the changes.
