How can I set PASS / FAIL criteria against a multi-choice question and a single choice question?

I created a demo form that will show Pass or Fail depending on the choices of the user using the Single Choice and Multiple Choice fields.

I have a few questions about the quiz add on:
- when a new question is added in the quiz, how can I get it to display in the custom email page?
- I have questions that are not a single choice or multi-choice question, but a Multi-select Grid. Is it possible to set default answers for this using the quiz add-on?
- when I receive the quiz answers email, the width of the table is overly large. How can this be reduced?

When you say "custom email page", do you mean to say notification email alert. Please note that if you have not edited email alert, when you add new form field in form it will automatically added in notification email alert. If you edit your email alert template, it becomes dirty and you will have to manually add the question in email alert. Alternatively, you can also delete your existing email alert and add a new one. Please note that a new email alert will always include all form fields.

Unfortunately matrix field or any widgets cannot be used in quiz app. Please note that only following field can be used in quiz app:

Radio Buttons Check

Box Text Input

Fields (Case Sensitive) Drop Down Box

Number Input Fields

Please note that you can edit quiz result's page template source code and change the width of table. Please check the screenshot below on how to edit result's page template:
