I created a form and I accidentally clicked the tag to the right of the form's title. Now the form has gone missing; it is not in trash or archives, it is not in another folder, and when I search for it, it is nowhere to be found. How can I recover my form and the response data? Thanks!

I checked the Page URL (https://form.jotformpro.com/62338084722961) you have provided and I can see that it is working properly. It means the form has not been deleted.
To find the form, log in to your account and paste the form ID in the search bar as shown in the screenshot below:
If you're referring to a different form, please provide the form ID or the website where the form is embedded so we can further investigate it.

I searched My Forms using the form ID and nothing comes up? Is there another way I can locate it? Thanks!

Sorry for the confusion.
The form (https://form.jotformpro.com/62338084722961) belongs to the account username (UCRGradDivision). I checked the main account and you do not have a permission to the form "Mentee Enrollment Form 2017" anymore. Here's what it looks like:
Please contact the main account (UCRGradDivision) and ask for permission. As seen in the screen animation above, the form must be checked so you can see it again on your account.

Our admin was able to find it easily and retag it. I now have access. Thanks for your help.
