Can I import a form from a file to put on my website?

    Our company provides Chiropractic Care and Physical Therapy to our patients. Each new patient must fill out a New Patient Form before they are treated at our practice for the first time. We are finalizing the new version of the form and would like to upload it to our website. We would like the filled out forms to be emailed to us after completion. Does JotForm offer this capability? Thank you. The page I have linked holds the current version of the form, as we have not yet added the new version.

    Yes we do have that, we also have an autoresponder which notifies the patient via email.
