Google Spreadsheet: Why is there some missing data?

O que faço para ter esses dados de volta? O que devo fazer, ja que o formulario não está informando o email nem telefone do contato.?.

I have manually push again the submission data to the spreadsheet just to be sure it is working.

Can you please check again the submission data.

If still not updated, can you share to us the URL of the google spreadsheet. You can set View Only option to avoid any writing on the data.
