Is this even possible? If so how?........

I am trying to create a form with a if/then field.

If they choose a certain date on the calendar...then it would link to a external site where than can sign a required waiver? 

Is this possible?

Yes it is . We can disable all other fields and skip directly to submit button which links to external url if a certain date is triggered.
