Jotform Account with healthcare organization

Hello,

I am working on getting jotform officially approved with our organization and they are asking for a contact person and contract information to get jotform set up.   Currently, we are trialing jotform and are paying via credit card, but the preferred method would be to set up Jotform as a vendor.

Please let me know what options we have.
