How do I export as PDF with images included

I have created a feedback form with an image inserted as a header but when exporting as a PDF I only get the raw data. I need the PDF to display the image header on the document. 

I have tested the form and I was able to replicate the issue you had. In order to enable your PDF to have images what you need to do is to enable headers and texts in your form submissions view. Below is a demo on how to do so.
