Why publishing to Facebook is not working properly?

I am trying to publish a form to a Facebook page but can get no further than the 'add jot form tab' button.  Nothing happens when I click on it, i.e. no pop up.  Would this happen if I had 'pop ups' disabled in my settings.  I'm using a 2014 Mac on Safari.

The popup should appear when you click "Add JotForm tab". I just checked and it works as intended. If it doesn't, then you might have it disabled or blocked. Did you try using any other Browser, Chrome or FireFox?
