conditionally shown fields are not showing as expected

As it has been built, it shows that there is a space to fill out a name and a church.  However, when people go to fill it out, the names for the 3rd, 4th and 5th names are missing.  I can't figure out why they are not showing up.  

I cloned your form to test it and it looks like several of the conditions are associated with fields no longer in the form.  Since there is not currently a name or church field for person 2, I assume those were the fields that were deleted:

Add the fields in for person 2, then update your conditions to include those fields.
