How to embed form inside another form?

I created several forms a year ago and they worked fine for a long time. I haven't needed them for a number of months, and now that I do, I find that they no longer work and the Jotform interface appears to have completely changed so that I no longer know how to make forms (I had gotten pretty good at it last fall). I need some quick help to make the forms work again.

There are three forms: A master form that gives people a choice of two options (one-time donation or recurring monthly donation), and then two other forms that come up depending on which choice is made on the first one. When I choose either one-time donation or recurring monthly donation from the master form, something strange comes up that looks like a small piece of the corresponding correct form, but not the whole thing.

Can someone point me to what has broken and how to fix it?

Apologies for the challenges you are experiencing. I think I know where the challenge is based on your explanation. One is the form layout has changed and you need to change the form layout to  "Legacy layout" from the current "New Layout". To do so kindly follow process as shown in the demo below:

Two I have checked your form and you are using and iframe widget and the default height and width of this widget is very small hence can not display the embedded form properly. You need to change the height and width to dimensions that display the embedded form properly. Kindly see demo below on how to do this.

You can also clone my form below

To clone form kindly use the guide below:

Hope this information was helpful. Thank you

I have attempted to change from New Layout to Legacy Layout, but there is no such choice is the settings for my forms. There is no Form Layout option.

Your form's layouts are ok. I checked both of the embedded forms and they are fine:

I increased the width and height of the "One-Time Donation" form:

You can do the same with "Recurring Monthly Donation" form and adjust the width/height as needed.

If you have any further questions or had something else in mind please let us know.

    Thank you!

Thanks. I tried to increase the Recurring Monthly Donation form size to the same as the One-Time Donation, but it continues to show up narrower in my browser.

I was able to see the issue you described when I viewed the form on the website you shared with us and even from its direct link. However, since I was not able to reproduce the same issue on a clone version of your form, I tried clearing your form's cache — How-to-Clear-your-Form-Cache and I believe that fixed the issue.

Please check your form again and let us know if you need any further assistance.
