How can I create an employee availability form?

    Hi Jotform, 

    i wish to create a simple weekly availability form for my staff. 

    I would like it to be very basic in that they just click a checkbox or something for the days or the week they are available...

    i cannot seem to find any templates etc like this or even similar that I can amend... can you please point me in the right direction if you even have something like this..

    If you are unable to find any similar template to the form that you wish to create I would suggest you to create your form.
