Internet Explorer stopped showing the form, it's just blank space. It was working previously, so I'm guessing there was a Microsoft update????? All other browsers show the form.

I checked your form http://www.jotform.us/form/62925327467161 in IE11, but I wasn't able to replicate this problem, the form is showing fine.
Is your form embedded in a website?
If it is, please share the URL so we can check.

If the current embed code you're using does not work, try it with an iframe embed code http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
If that does not make any difference then it's most likely the form is blocked or not working properly on your web page. Unfortunately, we can't give you enough details about the issue if we can't see the embedded form within our browser.

I tried the iframe and that still didn't work so then I did the pop-up and that worked. All other browsers are fine expect MICROSOFT...grrr. So I'll just leave it with the pop-up code and call it good.
