I've added a credit card payment to the form and I've noticed it's not displaying the title I gave it - instead it says Credit Card Information. I've attached screenshots below.
This is how it looks in build mode with custom title and credit card details...
This however is how it looks when you preview the real form. It takes off my custom title/description and also separates the credit card entry making it one extra step for customer.

I have been testing your form and I can replicate the issue you're experiencing, it seems to happen only with your form and while using the new form layout, I have tried several things on my end such as delete the payment field, log out, clear my forms cache, deleting products and even disabling the label for the payment field, but nothing worked.
I decided to forward this case to our second level so this is fixed and prevented to happen in the future.
We will update you as soon as possible.

Sorry for the inconvenience, you are right about the problem and I can replicate it on my end. We have started to work on the issue but I can not give you ETA for it. Until that time I can offer a temporary solution.
When I first examine your form, I realize that your language settings are set. If you unset it, your form will work as expected.
In order to do that, first, go Settings tab. Then click show more options button.
Then click X icon to remove language setting.
I will reach you from this thread when the problem is resolved.

We have fixed this issue. Your payment fields will be working as expected even if you add multiple language.
