Is there not a simple check box element you can add to a form?

Just a tick box  for an option for customers to sign up to a newsletter etc?

You can use the multiple choice field. This is a regular checkbox field which you can use for your form

You can also make use of the terms and conditions widget if your purpose is basically for the user to confirm and agreeing to terms of your service/website.
