    I have followed the steps above & headers & texts still do not print on the PDF form. I checked the "gear" and headers & text box has a check mark.

    Are you referring to this form ? http://www.jotform.us/form/61714386587164 . I have cloned that form and tested a submission then checked the PDF.

    I enabled the "show headers and text"
    I have followed the steps above & headers & texts still do not Image 10

    It seems to show headers and text correctly. Let us know if it is not the correct form you are referring to.
