How can I download the form as a PDF with all the info?

Hi, 

When I try to download the form, it only shows answers. I need to be able to download the entire form with all of the text included. How can I do that? 

This is for disclosures and consents for care. I see a lot of signatures but I need to be able to put this form in their electronic chart.

You have to access the form submissions page. Check the Show Headers and Text to include the texts in the PDF.
