Employee of the Month Voting Form

    I tried your employee of the month survey form temp;ate but the reports does not add up. See attached form, report and submissions information

    It's not possible with Dropdown, unfortunately. The visual report can only add single or checkbox selection.

    You may consider the checkbox type. That should work.
