In which platform did you create your website? Did you write your own codes yourself or did you use some platforms such as Wix?
Can you share your website URL that has this problem, so that we can check and investigate it?

I checked the page where your form is embedded and I can see the form adjusts accordingly when viewed on mobile.
However, there seems to be an issue with the responsiveness of the page where the form is embedded. I also tried removing the form on the page via source code, the responsive problem still persists. In other words, the issue isn't with the form but the page itself.
The standalone form is already mobile responsive https://form.jotform.com/72605241981153
Please ensure that the page where you have embedded the form is mobile responsive. You might want to check that in your adobe muse page settings.
