Form does not show up in IE 11 browser

    The form was updated, now it does not show the update in any broser and in IE11 does not show up at all?

    Parent was updated to show First Name and Last Name fields.

    Please Help, thank you.

    What version of internet explorer are you using? I just checked your form, and it loaded just fine:
