Field not populating properly or displaying in reports

    The "Departure Date" and "Arrival Date" are not populating properly in the form.

    If data is entered it is not translated to the reports.

    I have been checking the embedded form on the webpage you provided and noticed that you're using the source code. 

    Testing the direct link to your form seems to be recording the date fields properly:

    I would recommend you to please update your source code in order to have any update applied to the embedded version as well. 

    Please also avoid to change the fields names of your fields since that may cause the info to not be recorded properly.
