We’re having trouble with a form that we built producing two types of confirmation emails to the customer. When I built the template I used the prior template as an example and overlaid information on that to match what I was looking for on a form. Can someone contact us to correct this issue?

Can you please be more specific? What exactly do you want us to correct?
1. If you would like to use the same email template you can copy paste the html/css code
2. If you would like to remove your customized template just delete the notification/autoresponder and add a new one. It will automatically update with the default template.
