Form is not working

The conditional settings and page break is not working on my form named Order Form.  After I created a clone of the form to use for making changes, the conditions from the checkbox fields, for example on page 1, do not work to show the conditional field.  When I made the clone, it resulted in over 40 conditions and I am thinking this is too much for the form to handle.  Did it corrupt my form or is my account disabled?  I have deleted the clone and also made a test form to see if a new form would allow the conditional field, but it still is not working.

Your account is active and the form should not be corrupted. 

Were you testing your conditions in Preview mode? We had an issue earlier today and the conditional logic was not working inside of the preview mode and on the embedded form. This issue is now resolved. 

Please test your form again. If the conditions are still not working provide us an ID of your form so that we can test it.
