Is my account currently live. One of my forms seems to have gone down?

Your account is currently active. Are you having the issue with some specific form? Could you please provide us with a form link?
Also, it seems that you currently have two active subscriptions associated with your account:
1) Silver Monthly - $39.00/month
2) Bronze Monthly - $19.00/month
Which subscription would you like to keep? We can cancel the other one.
