I had 12 submission and now it's just stop working all of a sudden.

I checked your form using the direct link and the custom URL. I can verify that the form is working properly. Here's a screenshot:
Are you still experiencing this problem? I suggest you check it will work on another browser or not. You may also clear your browser cache and verify if that helps or not.
Lastly, I cleared your form cache so that your forms will be refreshed from our servers.
