Hello. I have finished and updating my first form on build but if I try to preview the form this is all that comes up.

I am able to replicate the issue on your account when I preview the form.
However, I am not able to replicate the same issue on my cloned form.
It seems that the issue is related to the custom name. The same issue can be replicated on the live form when using a custom URL - https://form.jotform.com/Josteen/dailies
However, when using the link without custom URL all fields are shown properly - https://www.jotform.us/form/73095935487168
Please reset the Custom URL and re-create it to see if that will resolve the issue.
You can also try to use a different custom name instead of 'dailies'.
