Form does not integrate with Google sheets

We created a form and integrated with Google Sheets.

The first time we fill out the form, it works.

The second time, it doesn't and we have to re-integrate form. Then it works...just once.

I have just checked the form on your webpage and found that it's currently integrated with Google Sheets, I sent the missing submissions to the integrated sheet, but this does not fix any error on the current integration. 

Please note that you may need to re-integrate again in order to have a fresh integration. Also, avoid to make any change on the integrated sheet since this will break the integration, we do recommend to make a copy in case any change is needed.
