I am unable to forward my new client information form to a new client. Can you explain why.

I am not sure if I have understood your question correctly. Do you mean to say that auto-responder email is not being sent from your email?
I did test your form and I am unable to replicate the issue you are having. I did receive auto-responder email successfully. Please check the screenshot below:
I would suggest you to please test your form again and get back to us if the issue persists.
