Form Completely Disappeared

A form of mine has completely disappeared in front of my face.

Folder Clients > Misc

Form > Maby - Order Form

Tried to disable it .. started to loop the process

pressed F5 > Gone

Not traceable through search

I notice that history states that I deleted this form. I did not delete it. i disabled it. if I deleted it ... why is it not in trash?

This is a 'live' form for a client.

Can you advise me please?
