Why does it say my account is suspended?

Your account is suspended because you are collecting Credit Card inform using the plain text fields on your form and this is against our Terms  of use. 

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately. 

I also think that you are aware of our Terms of Use because you are using shortcuts for the names of the Credit Cards and for other Credit Card info. I also found your other account that is also suspended for the same reason. 

Your accounts will remain suspended.
