What size (pixels) should I use in creating a custom graphic for a header?

It really depends on you, when having higher pixels the size of your header would be big and would slow your form loading. Having a very small header size will pixelate your header and it won't look good if stretched. Try experimenting in between, when you need to adjust the header you can contact us and we would be willing to assists.

My current header is too small and looks weird. I was hoping there was a
guideline for basic graphic to go across the top of a form.

In your Form Designer, since the "Form Width" is 690px.

You can change the Dimensions of your Header to 690 (width) which is also in PX.

I have cloned your form and applied the adjusted width/size of your Header.
