Currently the my order form will not process past the second screen option. How do I trouble shoot the form to restart?Page URL:

We had an error on our end that was causing a bug with email fields. The error was resolved a few hours ago and the form should now be working. I cleared your form caches just in case the bug was still present in your form.
Let us know if the issue is still present and we will be happy to have another look. i checked your form though and at this time everything appears to be functional.
