How can I copy a field from one from to another

I am try to add a birthday picker widget to a form but every time I try the system creates a new form.

I tested the Birth Date Picker widget and I was not able to replicate the issue. I cleared your form cache on our server. Could you please try it again and let us know if the issue still persists?
