My form keeps disappearing on mobile or tablet

Hi Jotform,

My form is disappearing on different breakpoints. It works on desktop, but when i look up the form on the my tablet and mobile it doesn't show up....here is the link

Also, the file upload is not showing up online either for some reason. Thank you

The form script didn't load properly when viewing it on mobile. It could be due to script conflict.

Please try to re-embed your form using iframe embed code. This might fix it.
