How to change form Layout?

How do I get RID of the new form---that break up... hate it  can-- I go back please..?

By "new form that break up" I would assume you mean the new layout design that sets every question in one page. This layout is called Jotform Cards Layout.

In order to change the form design layout, kindly follow the illustration below on how to do so.
