Light box feature not showing up form properly

Installed a form on our website. While standard form is appearing great (http://www.digitalsplendid.net/get-a-quote/), the light box version (link attached twice on the home page 'Get a Quote') not showing up properly. Help will be appreciated. 

I checked your website and it looks like you have two lightboxes embedded there (not sure why), since when I click "Get a Quote" a white box is opened and then after closing it a form is there:

Please try removing whole code related to that Lightbox and then re-add this one:

Thanks for your help. I placed twice the code, first at the top of the page and second at the bottom of the page. It appears that the lightbox code can be placed only once on the page. So, I removed the bottom one and  now it is working excellent.
