I created a form and I cannot find it in my account.

Hi I created a form and attached it using Trello Power-up for linking the form with Trello card. However, I cannot find the form or the submissions in Trello or JotForm.

The name of the form is: Booth Selection Form BGTS 2018.

This is a time sensitive form and I need to use it ASAP. If you could respond as quickly as possible, it would be greatly appreciated.

I have checked your account and I can see you have not any form there, I have also searched using the name you provided and I was unable to find the form you're looking for. 

Did you create the form from Trello? 

I think you may have created it as a guest account, if you have the form's link, please share it with us so we can search further on our end. 

If you received submissions via email you may also provide us with the address so we check on our records and find the related form. 

I also found a form named "Booth Selections for 2018 CPBGTS", but it belongs to the user dfuchsjr so if you recognize the account and/or you have access, please check there, otherwise you will need to contact the form's owner in order to have access to the form.

However, he cannot find the form in his account either.
I edited through Trello and not sure if I saved it correctly. Is there a way you can send Don or I the link for the form you found, this is the form we need.
