Why my form appears to be offline?

My form screen is coming up as just a white screen, on all computers. 

Do you have any idea why this would be?

I can see the form coming along nicely.

...and now it is working. There was something up as i didnt do what you suggested and refreshed the page. 

I had tried on several computers.
