I notice this was an issue for some users in December 2016, but was reported to be fixed. I am forced to use the multiple choice checkboxes because the radio buttons may not be "de-selected" by the user once chosen. However, this will not be a good work-around if the Entry Limits feature is not working.

I checked your form and found that the entry limits is turned off. I then tried the limit on my end using the multiple checkbox element. The limit is working as expected.
Kindly try enabling the Entry Limit option again and test it on your form. Let me know how it goes.
