I need to revert my form to a version that I can't find

I was using the Field Manager app to combine some fields between my New Member Intake Form and the Child Intake From.

I selected a bunch of fields on the New Member Intake Form and then wanted to unselect them. I mistakenly clicked "delete selected" thinking it would "unselect" them. However, I quickly realized that what I did was NOT GOOD. So, I stopped it in the middle by closing out the window, but damage was already done.  

I went to my revision history and it had a ton of revisions done today and then the next selection was my revisions on 12/31/17. So, I chose to revert back to that version but it made my form blank.  I was then able to re-revise to the version with my fields deleted but that is not my complete form.  How can I get my form back to the way it was yesterday?

One of the fields that I know is missing is:

"CURRENT Health Challenges. Please check each of the diseases or conditions that you are CURRENTLY dealing with, if any. While they may seem unrelated to the purpose of the appointment, they can affect on your care."

There are a ton of conditions that are tied to this question in particular, so I'm REALLY hoping that we will be able to get this back.

I have checked your form and I can see a version where all the fields seem to be present: 

I can also see that the field you're looking for is already there so did you restore your form to the desired version? 

Please let us know if the issue persists, so far the revision history tool is the only one to restore your form to a previous version.

I was able to restore it to a version that had most of the fields. Then I redid the missing fields. I’m hoping that I restored every missing field; so far it looks ok. Is there a way to revert back to how it was yesterday just in case?

Unfortunately, you can only revert your form based on the available options in the Form Revision History tool. If it is not on the options, then I'm afraid you cannot restore it to that specific version.
