I have tested the form, and the phone number is there. You seem to have figured this out already. Let us know if you still need further assistance.

I didn't do anything with the phone number field. I got the test from you and I see the phone field. What would cause the phone field to disappear?

I only checked the form email template including the field names in it. Well, it triggers the autosave without me changing anything, so perhaps that was just a cache, and it has fixed the issue. Anyways, let us know if the problem happens again so we can investigate further.
