I sent a survey to a client and she received the error message that is attached to this inquiry. Wgat does it mean and how can she get through it?

You do not have any form created in your account, but I saw that you are a sub account user of another account.
Please provide us a form link, so that we can test it. If the form is embedded, provide us a webpage URL.
Also, tell us when was that message shown when the form was opened when the form was submitted.
You can suggest your client to use some other browser or some other device.
