WE have recieved submissions from this but it has suddenly become inoperable with the Name field becoming a drop down instead of an entry field

I checked your form and the name field is text boxes, though the input font color is quite light:

The Name field did not change into a Drop-down field. You're seeing is the autofill settings of your internet browser. If you're using Google Chrome browser, you can disable the autofill by following this guide: https://support.iclasspro.com/hc/en-us/articles/218569268-How-to-Disable-and-Clear-AutoFill-Info-in-your-Browser

You are correct. however font seems to be unreadable so it looks like it is
not filling in. Was OK previously not sure why it has changed. How do I
darken the text. Sorry to be a pain.

You can change the font color. Please go to Advanced Designer, click on the input area and change the font color to your desirable color. Follow the video below for more information:
