Website form will work, Mobile one won't

Whenever I try and use my mobile device to fill out a client form it won't work. It will redirect me to a different page on my website. example ;I click in to fill my name and immediately will bring me to "before and after page" I used the Square space third party code and it did not work. I used  Frame and that didn't work!
