Login Failed - Account Suspended

    I received an email regarding a guilty field on one of my forms, and that the field needs to be deleted to reactivate account. 

    Please delete field or allow me access to delete field so I can resume using my account.

    Upon review of your account it was found that indeed your form:

    http://www.jotform.us/form/62576339988174 is collecting passport information.

    Please be advised that you need to do the following to comply:

    Add:


    1. An agreement stating the usage of sensitive information. You can use the "Text" tool. You may also use the Short Scrollable Terms widget or Terms & Conditions widget.


    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.


    If anywhere possible, could you update your email to a company domain? We will appreciate it.


    We reactivated your account and your form is available so you can make the appropriate modifications.

    I have removed the Passport Information fields on the form. Thank you.
