Sinlge choice or multiple choice check boxes don't work.

    Hi, I've used the same form for about 7 years. Recently my clients have been telling me the form is not working. Upon testing it in several browsers it appears to not work. All the fields fill out correctly but the check box above the submit button will not select . I tried editing the form and deleting the single choice check box and creating a new one. Even the multi choice box does not work. All I want is a simple check box at the bottom of the form for acceptance of terms. I have deleted the item off the form. Without the checkbox the form works fine. I will leave the checkbox off until I can get some support from you guys as the form is unusable with it on. Its important to note that I was not using any conditional arguments with the check box.

    I checked your form and found out that there is only a Text field there:

    Probably the best way to go would be to add Terms widget:
